BASW Cymru Practice Workshop: What does a smooth transition for young people with learning disabilities look like, as they move from child to adult services?

The National Institute for Health and Care Excellence defines transition as "a purposeful and planned process of supporting young people to move from children’s to adults' services”….” There is evidence showing that this transition is often problematic and a source of anxiety for young people and their carers.”

IMPACT worked in partnership with Pembrokeshire County Council who wanted to shine a spotlight on their Transitions service for young people with learning disabilities. They acknowledged that improvements could be made and wanted to explore how to ensure a smooth experience for young people as they move from Child to Adults Services.

At this session, we will share some of the insights and key priorities from this work.
